Aetna Insurance can refuse to insure you based on your enrollment application. Before you spend your time filling out an enrollment application, take a few minutes to call us. We can spot some health conditions you may have that Aetna will reject. We will also help you find the best plan for you and your needs. After talking with us, we recommend you fill out all the enrollment application and mail to us. We will review your application for missing information and will help you get insurance with Aetna. If the phones are busy, leave your name, phone number and a short message and we will get back to you shortly.

  Here are some helpful tips on the application and enrollment process.

  


1. Complete and sign the application to the best of your ability. When you mail the enrollment application to us we will review to see if there is anything missing that may stop your application from being processed.  
           
  2. Complete all questions of the health history on pages 2, 3, and 4. Only answer the questions for those family members who are applying for coverage. We will prescreen application with Underwriter if needed.  
           
  3. Question H, Page 4. We do not recommend you check this box. Let the Aetna underwriter tell us what they can do for you and your family first.  
           

 

 4. You will be asked for an effective date on page 4. You can choose the 1st or the 15th of the month. We highly recommend you keep your current health plan untill you are approved by Aetna health insurance.  
           

 

 5. Be sure to complete and sign the underwriting authorization on page 5.  
           

 

 6. You have many payment options available.(page 6)

The Easy Pay option allows you to pay the down payment from your checking account. (No down payment.)

The Credit Card option allows you to pay the down payment from a Visa or MasterCard charge card. They will take the payment from your charge card once your application has been approved. (No down payment.)

Direct Bill requires you to put one month down. Enclose a check payable to "Aetna Insurance ".

There is no charge for using Creative Health Insurance. The price of health insurance in Connecticut is fixed by law. You cannot find a better price anywhere for the same product. Generally, the insurance companies will pay commissions of 2% to 5% for a CT health broker to help you.

Sometimes we are able to find you the plan you wanted at a lower price.
