Here are some helpful tips on the application and enrollment process.

1. Complete and sign the application to the best of your ability. This is not a test, but when you mail the enrollment application to us we will review to see if there is anything missing that may stop your application from being processed. (Many people fill out the application as best they can and then call with any questions.)

4. You will be asked for an effective date on page 4. You can choose the 1st or the 15th of the month after the date you sign. We highly recommend you keep your current health plan untill you are approved by Aetna health insurance.

5. Be sure to complete and sign the underwriting authorization on page 5. If you are applying for a child under age 18, you as the parent or guardian would sign page 5. (In Connecticut, a minor is someone under the age of 18.)

6. You have many payment options available.(page 6)

The Easy Pay option allows you to pay the down payment from your checking account. Easy pay allows you to request all future monthly payments to be deducted from your checking account. (No down payment is required for easy pay.)

The Credit Card option allows you to pay the down payment from a Visa or MasterCard charge card. They will not take the down payment from your charge card until your application has been approved. (No down payment required) But you will be required to choose the Easy Pay option or the direct bill for future payments

Direct Bill requires you to pay the monthly down payment. Enclose a check payable to "Aetna Insurance " for the first months premium. Thereafter Aetna will send you a monthly bill.

7. Aetna requires the original application to be filled out in blue or black ink. Mail all the paperwork to:

There is no charge for using Creative Health Insurance. The price of health insurance in Connecticut is fixed by law - you cannot find a better price anywhere for the same product. The insurance companies will pay us commissions of 2% to 5% of the monthly premium to help you find the best medical plan for you and your family. This includes reviewing your plan when needed and helping you when we can.

Sometimes we are able to find you the plan you wanted at a lower price. This is not because we have special prices. We can do this because we specialize in health insurance here in Connecticut.
