How to Enroll with Anthem Blue Cross Health Insurance |

| Applying for Anthem health insurance is easy. However, Anthem can refuse to insure you based on your health application. It is recommended that you fill out the enrollment application and mail them to us. We will review your application and makes sure it is completed fully. If you have any health conditions, we will prescreen your application with the underwriter before sending to the company. This saves you time and helps you avoid being declined or rejected. | Print the enrollment application here: View & Print Application forms | |||||
| 1. Complete and sign the Enrollment/Change Aplication. (2 pages) Be sure to select a primary care physician. These forms are to choose your doctor and to issue ID Cards to you and your family. | |||||
| 2. Answer all the questions on the Individual Health Statement. (3 pages) Be sure to answer the questions for all family members who are applying for medical coverage. | |||||
| 3. If you want to pay Anthem directly from your bank in the future, fill out the Electronic Funds Transfer #7. Be sure to include a check or deposit slip marked "Void". (No signature required on check) | |||||
| 4. Anthem requires a down payment. Please include a check payable to Anthem Insurance. | |||||
