Applying for Connecticare health insurance is easy. Print the application package and follow the 5 steps. below. View & Print Full application package. Please dont hesitate to call us if you have any questions or need help. Most people fill out the applications as best they can and call us with any questions they find. |

| 1. Complete and sign the application/change form (part 1). Be sure to select a primary care physician. This form is to choose your doctor and to issue ID Cards to you and your family. |

| 2. Answer the questions on the Individual Health Statement (part 2). Be sure to answer the questions for all family members applying for coverage. |
| |
| 3. Complete and sign the Underwriting Authorization (part 3). |
| |
| 4. Enclose the first months premium payable to Connecticare. |
| |
| You can :A. Make a check payable to Connecticare or |
| |
| You can: B. Make your first payment by charge card. Connecticare will charge the down payment, only if and when accepted. |
| |
| If you want to pay Connecticare directly from your bank in the future, fill out the Electronic Funds Transfer Form. |
| |
| If you would like us to call on your behalf and help you get health insurance, sign this form. This allows Connecticare to talk with us about your personal health information and to help you enroll with Connecticare. This Broker authority stops once the Health plan is issued. |

| Connecticare will provide medical coverage for a Domestic Partner. If you qualify, fill out this form. |
